You Are What You Eat Essay
You Are What You Eat Essay You are what you eat is a phrase a lot of people use. Im not sure who came up with that phrase but it is very true. Food is a big part of what bring people together here in Yap. Most gathering on island during celebration looks colorful because of all the variety of local food, including fruits and vegetables. The people of Yap are friendly and they have the habit of offering food and drinks to visitor that comes to their place. It is very offending if the visitors do not except the food that was offered to them. Nowadays people would still offer food to visitors but what they have is not healthy food. Most are ashamed of eating to eat healthy food; a can of tuna is better than crabs. This attitude also transfers to all a lot of children, most would rather have luncheon meat, hotdogs, corn beef, or even turkey tail in as their food rather than fish, crab or clam. So, In addition, most of the foods eaten here in Yap are store bought food because it is that is fast and easy to eat. It may taste good and it is easy to make, but is modern food good for us? When ask an individual they would tell you that it is not. When an individual is asked, he or she would say it is not good, but what is said and what is done are two totally different stories. What they say and what they do is two totally different stories. How we eat and what we eat shows people how we were raised and what kind of person we would turn out to be. A person People who watches what they eat and does exercise is are more likely to be healthy. A person eating junk food who sleeps a lot and does no exercise would probably be obese and more likely to be diabetic. In the real world, it is not easy for a child children to stick with a healthy diet because what they see they copy. Parents who mix sugar in their drinks have children that also do the same thing. It is also the same with alcoholic parent their children are more likely to be alcoholic. It is up to the parents or the person taking care of the child to play a big role in not only making healthy choices for children and teaching children to make healthy choices for themselves. Before children likes to swim, play tag with other children or even jump rope but nowadays you hardly see children doing that. Most children spend their time playing games or watching television and eating junk food. The advertisements on television does not help children to eat better. What the children sees on TV to them looks delicious and appetizing, so they would pester their parents to get it for them. If parents or the person taking care of the child is a lenient one, the child would get what they he or sh e wants. The hospital can also help with the support and information the parents and caregivers needs to make healthy choices while instilling healthy eating habits in children that will last a lifetime. Health providers should help the communities to understand the consequence of eating imported food. Long before the foreigner came, people teach by showing examples and explaining things. The old teaches the young what is good to eat and how to go about fixing it. No one worries about gaining weight having cardiac problem or even having high blood pressure. People only eat local food and they live long and stay healthy. Today our way of leaving is declining because of all this easy to get food from the store. We have doctors, nurses and scientists telling us how to manage our lives. They also tell us to go back to the way we used to eat, which is good but hard because most of us grow up with modern food or imported food. When people attend a celebration or even a small gathering, food is one of the things people like to share but instead of eating local food, they would eat imported food (Needs Assessment for Micronesian Health Care in Hawaii 13). Today with people coming back from school with all this idea in their head on how to better help the people of Yap States. They go aroun d saying people should not do this or do that because of too much sugar, instead of showing and saying that they can eat this kind of food, but not all the time. Even in the hospital, nurses sometimes give wrong information to inpatients, especially to diabetic once. Some nurses would tell the patient not to eat certain local food because it contains sugar. In reality the patient can eat the food because it is local, but not all the time. One of the things that help people in Yap long time ago to stay healthy is exercise. Everything Yapese people do is a certain kind of exercise. Even going from places to places people either walk or go by canoe. Getting firewood is also not an easy task but the women also do this kind of work. They cut and gather two to three baskets of firewood for their own house. The women go to taro patches clean and harvest the taro which is not an easy job. Sometimes harvesting taro for the family a women would do that two to three times a week with one to two basket of taro. Sometimes they would climb up or look under the coconut tree for leaves to cut and weave it into basket for the taro. They also do gardening and most of the time the males do not help. They cut small trees, vines pull up grass or weeds and they even burn down big trees so that they can plant seeds underneath. They would carry the taro home and also cook it. They take care of cooking for the whole family, cleaning the house , and taking care of the children with the help of the young girls if the family has any. The ladies do not have that much free time to hang out with their neighbors because almost everything around the house they are responsible for. Girls have the responsibilities to help their mothers with the chores around the house. This paragraph needs to be written in the past tense as it is a discussion of the past As for the men fishing is one of the things they do for their families and relatives. They would either go spear fishing, netting, using lines or trolling by boat. Trolling and line fishing is more relaxing then netting and spear fishing. Netting and spear fishing is one kind of fishing that deals with swimming or diving around in the water. For the women only few of them goes fishing for their families but the rest do not. The men also have the responsibility to build houses for his family or help his extended families with their houses. Sometimes, the men would help with preparing the garden. If the men are not fishing they are building canoe or teaching young boys how to build things. Most of the time, the men are the once who climb coconut tree to get coconut for the family and they also climb and harvest betel nut to chew. The boys help with the climbing and sometimes the girls if the family does not have boys. Past experience also The pacific island countries are in a state of transition. Pacific island people are changing very fast from a lifestyle which is very healthy, with farming, fishing, walking and gardening, to a less healthy lifestyle where food and goods are purchased and not produced locally, people use cars, and generally there is less physical exercise. The increased migration from villages areas to towns and cities has also meant a big change to the pacific way of life. Today people do not have enough exercise they spend most of their time watching television and when they want to go somewhere they go by car. Climbing coconut is another way of exercising which few people are still doing it. The taro patches is over grown with weeds because people are too lazy or do not care to clean them. People have the tendency eat and lay down to rest or sleep claiming that the food they just eat need to digest. Other people would jag around the lagoon and comes back and eat whatever they want. The only exerc ise most people do is to walk from the house to their car. Parents have the tendency to over feed their children and by doing this they create a problem. First they start introducing food to the baby when they are a few months old because to them the baby is not having enough food with breastfeeding. Second is when the baby is a toddler, the parents would spoon feed the toddler to the point that the toddler with start to refuse the food and the parents would force the child to eat all the food in the plate. Right at that minute the parents create a problem, forcing the child to eat more than the childs body needs. Too Yapese a fat child is considered healthy and a slime or skinny child is considering sickly and needs to fatten up. Health worker should encourage and explain to the parents that babies should only stick to breastfeeding for the next six months. After the six months, when feeding the baby they should not over feed them. When the baby is full the parents should stop feeding. By doing this the parents help the child to eat only wh at their body needs not what they think their body needs. It is also good to start giving local food to your child and not store bought baby food. Also, the parents or caretaker should stay away from giving coconut milk to new born it is full of fats. The doctors, nurses, and health worker should advice this people to stay away from coconut milk. Even though Yapese people believe that giving coconut milk to babies is very healthy and also feeding them that young is good for the mother and babies. Healthy people long ago only drink plain water and coconuts. Those are the two kinds of drink that they drink and it is good for them. Plain water or coconut without ice and it is not cold. Today people bought soft drink from the store and other things to mix with their water including sugar. They even bought alcohol like beer and other beverages to also drink. People do not have time to look for coconut and if they do they harvest them to sell not to drink at the house. Coconuts are turning into copra because only few people would drink them. For those that they want to drink they spend money looking for them and buying them. Coconuts are getting very expansive to buy nowadays. So, parents mix their childrens drink or by them juice from the store and send them to school. Some even buy soft drink for their kids because to them it is cheaper than buying coconut. According to Maria Laaken, one of my colleagues at the hospital children nowadays wont drink plain water because it does not taste good and they dont want to be different from the other kids. At the same time the parents dont want to deal with all complains early in the morning before going to school. Yap is well known for having many good healthy local foods. It is the people who made bad choices when it comes to food. All ever the island you will see variety of local food including vegetables and at the same time some of the stores also sell local food and vegetables. It is easy and possible to grow them because Yapese people also own land. It should be foreign people or people from other places who should have problem with their diet not Yapese. Instead foreign people go around looking for all the local food and the Yapese goes look for all the imported food. Most taro patches here in Yap are overgrown with grass or weeds because people like to eat white rice. Inside most ice-box of people here in Yap is full of turkey tails and chicken, it is hard to find fish. Overweight in the Pacific Professor Susan Cassels claim that all of the foods that the people in the Federal States of Micronesia like to eat are foods that are easy to get or easy to make and it is affordable or a lot cheaper. One of those foods is turkey tails and turkey tail to the people in the United States is not food because to them it is fertilizer. To the Yapese it is one of the cheapest and tasty foods because of all the grease and fats. Increase availability of money and people is expose to imported foods have result in people not eating the right kind of food, and increase consumption, with diets high in sugar and fats and often low in fiber, vitamins and minerals. A reduction in physical activity, fishing, walking, gardening has led to an increase in obesity in both adults and children. The exercise researcher at the Pennington Biomedical Research Center in Baton Rouge, La. Dr. Timothy S. Church, the reason the population is getting obese and overweight is because of lack of physical activities at work, and those jobs that makes people to do physical labor are all gone. A high rate of smoking and increasing consumption of alcohol makes the situation worse. The eating habit of people here in Yap contributes to most of the diseases that the people of Yap encounter. Because of foreign influence and people wanting to be Americanize or modernize they would go to a lot of trouble to eat imported food. Before people hard ly get sick and when they do it is not life threatening. People do not have problem with their sugar level or have high blood pressure. Consumption of imported food contributes to a lot of people dying in Yap. All the non-communicable disease in Yap can be stop or prevented by going back to the way we leave. We eat only local food either from these or from the land and this sickness will leave our people and also our island (Jackson 88). There are many non-communicable diseases here in Yap. One of them is diabetic, also known as having problem with your sugar level. Diabetes develops when the body does not make enough insulin or is not able to use insulin effectively, or both. There are two types of diabetic, I and II. Diabetic type I it is believe that a person is born with that type of non-communicable diseases or it is passed from mother to child. As for the other type, type II diabetic a person gets it latter in life. In 2006 to 2007 there was a survey done on Yap Proper and it shows that in general 23% of the people are diabetic and 35% are hypertension (Ichiho, Yurow, and Lifang 16). People with diabetes can lead a normal healthy life. Diabetes is manageable by eating the right food, exercise and sometimes medication. Medication that the patient is taking does not cure the patient but only help to prolong the patients life. All people with diabetes and hypertension that need advice about diabetes and hypertensi on management can ask help from a doctor or nurse. The population of Yap Island has significant burdens of obesity and overweight, hypertension, and diabetes. Obesity and overweight appear in children as young as two years of age and skyrocket between ages 15 and 25 years in both males and females. Following the development of obesity, both males and females begin to experience hypertension and diabetes within 10 years. (Durand 163) It is suspected that this non-communicable disease happens because of all the imported food the people eat and drink. To prevent people from having diabetic people needs to go back to the way they live. Go back to eating local food from the sea and food from the land. Compare to the United States the Federal States of Micronesia has 20% more diabetic people (Needs Assessment for Micronesian Health Care in Hawaii 28). The other non-communicable disease is hypertension. There are two types of hypertension. One is Primary Hypertension and that is the most common form, accounting for majority of cases in Yap. Second is Secondary Hypertension and it occurs as a result of many factors one example is kidney disease. No one knows the cause of hypertension the only thing they know is the contributing factor and they are ageing, smoking, eating food with to much salt in it, being overweight or obese, drinking a lot of alcohol, not enough exercise, stress to name a few. To prevent people from having hypertension they should maintain a healthy weight because being over weight contributes to have high blood pressure. The other thing is people should get regular exercise because being physically active makes the person less likely to have high blood pressure. It those not mean you have to ran around the track or being in a marathon. It just means that people needs to move a bit like wash dishes, cooking local food, gardeningà ¢Ã¢â ¬Ã ¦etc. Many people in Yap are dying because of what they put in their mouth. Most dead is because of the amount of food people eat and also what kind of food they eat. The lifestyle of the people in Yap contributes to a lot of problem that people are facing getting fat and obese are two other health problem that the people of Yap encounter. Some end up with amputate limb because of uncontrolled eating ways others noncompliant once are dying each day. According to Jackson, the bad news about these diseases is that they are now killing Micronesians fastà ¢Ã¢â ¬Ã ¦ at least one person dies every second day from cardiovascular diseases in other words one person every other day eats themselves to death! Diabetes and hypertension has increase in proportion in our islands. In 2010 alone there are 847 diabetes and hypertension patients total that comes in for visits (Yap State Statistical Yearbook 148). This means that people in our island can and has suffer from complications from diabetes an d hypertension such as blindness, amputations and kidney failure. After having one of these conditions the individual will pass away eventually. There are a lot of things the hospital can do to help the people of Yap States to eat healthy and live long but that is up to the people to be willing to except the advice given to them. The doctors, nurses and health workers can only do so much but it is all depend on each individual. The doctors, nurses, and health worker can show people what would happen if they will not listen to their advice but if the individual is close minded than this really nothing the hospital can do. One solution is to keep to the traditional ways of living, eating and exercising including dancing. The people must also keep in mind that not only eating healthy food would save them but also exercises help. They also need to teach and encourage the public that eating healthy starts when people are young. They should not wait until it is too late. Because having a heart attack is not a laughing matter once a person gets it there gone. Unlike strokes and heart failure they will still see tomorrow but still th ey can also go anytime. Even kidney failure, having that is a drain on the families of the patient. The people should go back and start planting their garden again that is if they want to live long. They should also start cleaning their taro patches and planting taro. Be aware of diabetes and hypertension and what is going on about these diseases. People have to have protective measures to prevent the incidence of having hypertension and diabetes. Every year in Yap women and mens week are held to help doctors to detect any diseases and sickness that the people of Yap might have. It is held every year but it is really up to the people of Yap State to want to do it. Announcement goes out by radio, television and even flyer to encourage people to join. One should change ones lifestyle, so that one is more active and is able to either prevent hypertension and diabetes or, if suffering, learn to cope up with it. Having these kind of diseases is hard because there is no turning back once a person has it they have it for life. So, it is really up to the individual to decide what is better for them and their kids. Too either gets sick and dies young or lives long and have a healthy life.

Comparing Success in Horatio Algers Ragged Dick and the Life of Colin
Success in Horatio Alger's Ragged Dick and the Life of Colin Powell What does success mean to you? I think the idea of success is affected by the social system. In America and Hong Kong, which are capitalistic societies affected by the American Dream, success means money and fame. In other societies, success might have different meanings. Some people said money and fame is the true meaning of success, but I think that the true meaning of success is to follow the interest of your own and being good at it, but not money and fame. Overemphasizing on these two factors can only lead to negative effects. Therefore I think the definition of success in capitalistic countries should not be focused on money and fame, because it restricts people from following their real dreams. The idea of success in the capitalistic countries is mainly about money and fame. Capitalism, the social system of America, is based on the principle of individual rights, which is that everyone has the rights to do whatever they want without violating the law. Capitalism has an idea that the individual is sovereign. Therefore a man's thinking determines his choices and actions, and each man is the master of his own destiny. Every man in the capitalist society has the freedom of action, and those actions are necessary to support his own life. The capitalistic idea contributes to the formation of the idea of success because as each person has individual rights, everybody has the right and freedom to own their own wealth. Which is very different from socialism, in socialistic society, the people have to share their wealth with the government. As wealth can be own, people who have more wealth seem to be more successful, money becomes the main aspect of success... ... in the other hand, I think if people reach their goal for money and fame, the real dreams already changed. Overemphasizing on money and fame in success restricts people to follow their real dreams, and also give negative effects to the society. I believe that many people, like me, already changed their goals, and would regret for the rest of their life. Therefore, I do not think people should be fixed because of some social ideas, but should fight against these wrong, fixed ideas. Works Consulted Alger, Horatio. "Ragged Dick." Taco Bell Not Real Mexican Food in The Great Taco War :: essays research papers
RR - ââ¬Å"The Great Taco Warâ⬠The Real Deal Mariah Carey is one of the top pop singers in America today. On an average, her CDââ¬â¢s sell for fifteen dollars. However, many find it easier to purchase a ââ¬Å"knock-offâ⬠for a cheaper price than to purchase the real thing. For instance, an individual can purchase the exact CD at a local flea market for five dollars and save the extra ten dollars for something else. In the short story, ââ¬Å"The Great Taco War,â⬠Jose Antonio Burciaga informs readers that fast food industries like Taco Bell do no sell authentic Mexican tacos. He urges readers to purchase tacos from taquerias because unlike Taco Bell, their food is ââ¬Å"realâ⬠Mexican food. In the beginning, Burciaga provides a brief history when Taco Bell was established. First starting in Mexico City and then spreading throughout the United States, the chain sold ââ¬Å"mild imitations of the real thingâ⬠(382). Many Mexican businesses and people protested against Taco Bell because unlike homemade tortillas made from hand, they used ââ¬Å"prefabricated hard tortilla shellsâ⬠(383) that tasted nothing like real Mexican tacos. Additionally, the restaurant also combines food and makes up names so that it appears different. From Enchiroto, a combination of a burrito and enchilada, to Cinnamon Crispas, known as bunuelos, Burciaga points out that ââ¬Å"the Taco Menu can be a mystery if one is not familiar with the renamed food itemsâ⬠(383). Lastly, the author informs readers that compared to Mexican food, Taco Bellââ¬â¢s food is cheaply made. Upon visiting a local Taco Bell restaurant, Burciaga orders a menu item and begins to consume the taco when he notices that the ââ¬Å"meat was lukewarm and the cheese and shredded lettuce were cold.

Symbollism in Goodman Brown Essay -- Essays Papers
Symbollism in Goodman Brown The Symbolism in Nathan Hawthorne's "Young Goodman Brown" begins before the story starts. The first sign of symbolism is in the title of the story. The word "young" is used in saying that Mr. Brown is young in his marriage and in his maturity. The next word "Goodman" was a term used in Hawthorne's day as a man under a gentleman. This fits Mr. Brown because he is no one special, only to his friends and family. Symbolism is strewn throughout the story in what seems like every word. In the beginning of the story, the phrase "Faith, as the wife was so aptly named" shows that the word "Faith" is not only his wife's name, rather it is also the "Faith" one must have to believe in religion. Mr. Brown's faith is tested in this story to determine wether or not he is good or evil. Secondly, when he is leaving and his wife is begging that he stay. This is awful close the situation later in the story where his mothers ghost is trying to hold him back. Yet, Mr. Brown tells his wife that "My journey, as thoust call it, forth and back again must be done ââ¬Ëtwixt now and sunrise." Taking a journey as she calls it like his father and his grandfather had when they were young. Once again, later in the story when his mother tells him to stay back, the ghost of his father is the one who tells him to go on. When Mr. Brown enters the forest he says that "There may be a devilish Indian behind every tree" (which will be discussed later) and "W...

Hacking is Stealing Essay
Hacking started in the ââ¬Ë70s when a group of friends decided to tap into phone lines and make calls for free. Those people were called Phone Phreaks, a group of young men who twisted technology and used their technological know-how to create simpler solutions to complicated problems. What started as an intrinsically good past time slowly became a form of pranksterism. The Phone Phreaks soon held phone conferences from everywhere in the world. However, Telephone companies were less than delighted, and phone tapping became a crime. As more and more Phone Phreaks got persecuted for these crimes, the movement stopped, and the Phone Phreaks were almost of the extrinct race. However, the sensibilities of trying to one-up learned engineers and big corporations did not die down. A couple years passed and the same Phone Phreaks discovered a new form of technology to ââ¬Å"playâ⬠with: the computer. And the exact same people who started and led Phone Phreak conferences were the people who started hacking computers, a new revolutionary product that everyone wanted to have. Back then, computers really did not have any practical purpose at home, but for a hacker, the simple joy of watching the screen light up, and looking at moving blobs of colors were enough. However, with the advent of the internet, everything changed. When the internet was first used, all the information was open for everyone to use and manipulate. However, with the internet slowly becoming a place of business and transactions, where a lot of personal and, supposedly, confidential information about people and companies are stored, hackers posed as a huge threat What started as a hobby of manipulating electronics to prank people and find easier solutions for common problems turned into something more serious. While Phone Phreaking in the 70s was encouraged as somewhat of an exercise of the mind, computer hacking was a far different story. Computer hackers did not stop at pranking people; they started to use information they gained from hacking government and corporate computers. Hacking became a crime. According to PCWorld. About. com, some people even rigged phone systems to win two Porsches and other prizes at a radio contest. This poses an ethical problem that has always been a problem in society since the beginning of time: stealing. No matter what mode they use, may it be as blatant as rigging telephone lines to win prizes, or as furtive as stealing an identity to escape prosecution by the law, it is still all considered stealing. Taking someone elseââ¬â¢s property, and using the results of someone elseââ¬â¢s hard work (without permission, and through deceit) to your advantage is very unethical, and is one of the most basic things society teaches us not to do, no matter how hard times may be. Somehow, hackers forget that what they do is stealing, because they are too focused on breaking down systems and trying to outsmart companies and electronics. Hackers are intelligent people, yet instead creating their own products and reap the fruits of their own labor, they do the opposite, and use their skills and abilities to take advantage of other peopleââ¬â¢s weaknesses.

But I firmly believe that, recondite down, theres a tiny part of Damon that finds how he was when we were together. And now he has to be worse than ever to prove that what he remembers was all a lie.So firearm youre recitation this,Damon I know youll get generate of it somehow and snoop let me tell you that you were nice for a while, actuallyNICE,and it was fun. We lambasted together. We even laughed at the resembling jokes. And youyou were gentle. And now youre going, Nah, its just other Elena-plot to get me to think I can turn around butI know where Im going, and I dont care. Does that ring a bell, Damon? Have yousaid those language to someone recently? And if not, how do I know them? Could it be th at for once Im telling the truth? Now Im going to obturate that youre totally besmirching your honor by denotation secret things that dont belong to you. What else?First I miss Stefan.Second I didnt really pack for this. Matt and I swung by the boardinghouse, and he grabbed the money Stefan left for me while I grabbed an armful of clothes out of the jam heaven knows what Ive got Bonnies tops and Merediths pants, and not a decent nightgown to my name. But at least I also got you, precious friend, a present Stefan was saving for me. I never really handled type in a file mark Diary anyway. Blank books like you are my style.Third I miss Stefan. I miss him so prettyly that Im repetitive while Im writing about clothes. It looks as if thats what Im crying about, which makes me seem insanely shallow. Oh, sometimes I just want to scream. ordinal I want to screamnow. It was only when we got back to Fells Church that we found what horrors the malach had left for us. There is a fourth litt le fille I think may be possessed like Tami, Kristin, and Ava I couldnt really tell, so I couldnt do anything. I have the feeling that we definitely havent perceive the last of this possession thing.Fifth But worst is what happened in the Saitou house. Isobel is in the hospital with raging infections in all her piercings. Obaasan, as everyone calls Isobels grandmother, was not dead as the first paramedics who got there thought. She was in a deep trance reaching out tous . Whether some of the courage I got, some of the judgement in myself, was really due to her, is something Ill never know. But in the den was Jim Bryce. He hadoh, I cant write it. He was the schoolmaster of theb asketball team But he hadeaten away at himself his whole left hand, most of his right-hand fingers, his lips. And he had put a pencil through his ear into his brain. They say (I heard this through Tyrone Alpert, the doctors grandson) that its called Lesch-Nyhan Syndrome (sp? I only heard it said) and tha t its rare, but there are others just like him. Thats what the doctors say. I say it was a malach making him do it. But they wouldnt let me in to try to take it out of him.I cant even say hes alive. I cant say if hes dead. Hes going to a sort of institution where they keep semipermanent cases. We failed there. I failed. It wasnt really Jim Bryces fault. So he was with Caroline just one night, and from there he passed the malach to his girlfriend Isobel and to his little infant Tami. Then both Caroline and little Tami passed it along to others. They tried to give it to Matt, but he wasnt about to let them.Sixth, the triad little girls that most definitely did get it were all under the orders of Misao, from what Shinichi said. They say that they dont remember anything about decorating themselves or propositioning strangers. They dont seem to remember anything about the time of their possession, and they act like very different little girls now. Nice. Calm. If I thought Misao gave up e asily then I would be sure theyll be all right.Worse is the thought of Caroline. She was a friend once and now well, now I think she needs ease more than ever. Damon got toher diaries she kept her own journal by recording herself on video, and we watched her talk to the mirrorand watched the mirror talk back. in the main it was her own image that showed, but sometimes, at the beginning or end of a session, it was Shinichis face. Hes good-looking, if a little wild. I can see how Caroline might strickle for him and agree to be his carrier of malach in the town.Thats all over. I used the last of whatever Power I know I have taking the malach out of those girls.Caroline, of course, wouldnt let me near her.And then there were those fateful words of Carolines I need a husband Any girl knows what that means. Any girl feels sorry for another who says it, even if theyre unfriends.Caroline and Tyler Smallwood were going together until about two weeks ago. Meredith says Caroline dropped h im, and that grab her for Klaus was Tylers revenge. Butif before that theyd been catnaping together with no protection (and Caroline is dumb passable to do it), she could certainly have cognize she was pregnant and been looking for another guy by the time Shinichi turned up. (Which was just before I returned to life.) Now shes laborious to pin it on Matt. It was pure bad luck that she said it happened on the same night the malach attacked Matt and that that old man from the Neighborhood Watch saw Matt drive home and pass out at the steering wheel as if he were drunk or on drugs.Or maybe it wasnt just luck. maybe that was all part of Misaos game, too.Im going to sleep now. Too much thinking. Too much worry. And, oh, I miss Stefan He would help me deal with the worry in his own gentle but keen-sighted way.Im dormancy inside the car with the doors locked. The guys are sleeping outside it. At least, thats how were starting at their insistence. At least they agreed on that.I don t think Shinichi and Misao will stay away from Fells Church for long. I dont know if theyll leave it alone for a few days, or weeks, or a few months, but Misao will heal and theyll come back for us eventually. That means that Damon, Matt, and I were fugitives in two worlds. And I have no idea whats going to happen tomorrow.Elena
The Use of Lasers in Esophageal Cancer Treatment
esophageal malignant neoplastic disease is a rargon form of malignant neoplastic disease, unless its numbers argon on the rise nigh the world (Staff, Mayo Clinic 1). This disease occurs when malignant or pubic lo practiseous cells form in the tissues of the esophageal ocean liner of the physical structures gastrointestinal tract. The esophagus is responsible for(p) for moving swallowed food and liquids from the mouth into the stomach for digestion. It consists of some(prenominal) tissue layers, including the mucose membrane, muscle, and connective tissues. When cancer forms in the esophagus, it begins in the inner virtually layer of the tissue (the mucous membrane) and moves outward (towards the connective tissue layer) as it spreads (see count on 1). It often goes undetected for many years, and in its by and by stages, this cancer is non advantageously treat suit fitting. Among those who develop esophageal cancer, their long-term survival rate is low, as it is estimated that be fonts 12. 5% of patients live atomic number 23 years beyond the date of diagnosis (Triesschejin, Martijn 5).The standard discourse extract used to combat esophageal cancer is called an esophagectomy, in which the cancerous argona of the esophagus is removed via surgery, however this procedure is very invasive, complicated, and is often times fatal. However, with at at a times technological advancements, untroubledr, less-invasive methods are beginning to flip-flop this option and are proving to be successful in helping to extend the lives of those patients with severe esophageal cancer and aid in improving their quality of life. One energetic treatment option that is far more than safe and practical than esophagectomy is the integration of optical masers in photodynamic therapy, or PDT. Although it is electrostatic an experimental treatment, is ideal because it has no long-term side effects, is minimally invasive, can be done apace with little recovery time, has t he ability to precisely butt the areas of the body where the cancer cells are present, and can be safely repeated multiple times until the desired results are achieved.Post-treatment, it has been known to cause sensitivity to blowzy, peculiarly pertaining to the patients eyes and skin, however this has non been shown to be a significant issue among those do by with PDT. Photodynamic therapy uses special drugs, known as photosensitizing agents, along with laser baseless to kill cancer cells closer to the step up of the skin. The agents are injected into the body via and switched on with light of the admit wavelength depending on the drug utilized. Chemical and molecular reactions inside the ody from the drugs wherefore destroy the cancer cells and work to release the body of the esophageal disease. Although several sources of light are possibilities when performing photodynamic therapy, lasers are the some preferred. They are the most strong in emitting monochromatic lig ht, or light that is of a single wavelength and color, corresponding to all of the antithetic absorption maxima of photosensitizers compounds, and can be relatively easily coupled to optical fibers from 200 to 600 microns core sizes.Specifically, the eccentric person of laser that is optimal for the treatment of esophageal cancer is a rectifying tube laser. These lasers are the most fresh to be used in cancer treatment, and were real just within the last fifteen years. Thus, they live with higher quality beams, expanded ranges of wavelengths, and can be used on higher powers if desired. Diode lasers are ideal because they are simple to use, easy to transport, and do not require difficult installation exchangeable the gas (argon) and dye lasers used before them (Boucher 74).The diode laser currently approved by the FDA is manufacture by Angiodynamics, and has been successfully used, along with the correct lotion of photosensitizers, in esophageal cancer treatments. A effectu al photosensitizer is preferably a pure compound with a constant composition (Nyman 3). And in esophageal cancer treatment via PDT, the best-proven photosensitizer is called porfimer sodium, also commercially known as Photofrin. In fact, the United States Food and Drug Administration has however approved the use of this photosensitizer in the treatment of esophageal cancer via PDT (Photodynamic Therapy 2). Porfimer sodium is injected intravenously, where it is then absorbed by all of the bodys cells, but alone those cells that are normal and not cancerous are able to get rid of it. To allow for the bodys normal cells to rid themselves of the drug, two to 3 days are given between the drugs administration and the activation of the drug by laser light. After being passed down the throat by means of a small flexible tube called an endoscope, a thin fiber optic glass concatenation is used to direct the laser light at the affected area. The optical fiber is made of fused silica, and u ses a balloon catheter to distribute a defined, controlled gist of light to the treated esophagus organ. If the beam were not diffuse, a direct beam (even at a low power) could be damaging to the esophageal tissue. The light is operated at weaker power of around 2 Watts to take care that it does not burn any tissue, and it is relatively pain-free.The treatment it is applied for no longer than an hour at a time and can often be done in an outpatient setting (reference all sources listed). Normally, the most common wavelength used is red light, as its ad hoc wavelength of 630 nanometers has been shown to work best when attempting to create the most effective and potent malignant cancer cell-killing method possible. Atomically speaking, next its activation by laser light, the photosensitizer is elevated from a ground state to a long constant excited triplet state. and it can then react with cell membranes to form radical ions intermediates which interact just with oxygen to produ ce cytotoxic oxygenated molecules (Gray 1). more simply put, the excited photosensitizer activated by the laser light transfers its energy to other molecules before reversive to its ground state, which generates a reactive oxygen species (intermediate) that is able to directly kill and destroy the esophageal neoplasm via oxidation, and later, indirectly, via inflammation.Research has shown a huge increase in the survival rate of PDT patients receiving this treatment. Treated patients survival rates meliorate 25% over a span of five years compared to those who never received any emblem of treatment (Li 2). And although doctors have seen great success with the use of photodynamic therapy and diode laser treatments, there is still oft more to be researched and improved upon. Currently, new drugs, such(prenominal) as Photochlor, and others are being clinically time-tested to see if they can be used as photosensitizers in the treatment of esophageal cancer. This photosensitizer look s promising, as it has already been shown to last a much shorter time and is more easily removed from the body than Photofrin is. Also, scientists are looking for at the possibility of using ointments containing ferrous or colbalt ions and hydrogen peroxide on the treated cancerous area to improve PDT outcomes along with laser light. And with the further advancement of technology, hopefully newer treatments will be able to not only improve the outcomes of patients with severe esophageal cancer, but be able to cure them of it once and for all.
